Sinusitis is inflammation of the paranasal sinuses — the air-filled cavities in the skull around the nose. It is classified as acute (less than 4 weeks), subacute (4-12 weeks), chronic (more than 12 weeks), and recurrent acute (4+ episodes per year). It may be infectious (bacterial or viral), allergic, or non-allergic in origin. The maxillary sinuses are most commonly affected, followed by the ethmoid, frontal, and sphenoid sinuses. Chronic sinusitis involves persistent mucosal inflammation, often with nasal polyps, impaired mucociliary clearance, and microbiome disruption in the sinus cavities. It is frequently associated with allergic rhinitis, asthma, dental infections, and nasal septal deviation. Postnasal drip from sinusitis is a leading cause of chronic cough, throat irritation, and interrupted sleep.
Sinusitis arises when the sinus ostia (drainage openings) become obstructed and the mucous lining is unable to clear normally. Viral upper respiratory infections initiate most acute episodes; bacterial superinfection follows. Allergic rhinitis is the most common predisposing factor for chronic sinusitis by causing persistent mucosal oedema and impaired drainage. Structural factors — deviated nasal septum, nasal polyps, and concha bullosa — reduce ostial patency. Environmental factors including air pollution, dry air, and occupational irritants damage the mucociliary apparatus. Dental infections can seed maxillary sinusitis directly. Immune deficiency and fungal colonisation of sinuses are important in recurrent or treatment-resistant cases.
