Allergic rhinitis is a type I hypersensitivity reaction of the nasal mucosa to airborne allergens, mediated by IgE antibodies. When sensitised individuals are exposed to allergens such as dust mites, pollen, pet dander, mould, or cockroach proteins, mast cells in the nasal lining degranulate, releasing histamine and other mediators that cause the characteristic symptoms. It is classified as seasonal (hay fever — triggered by pollen) or perennial (year-round, triggered by indoor allergens). Allergic rhinitis is part of the atopic triad with asthma and eczema. Poorly controlled rhinitis disrupts sleep, impairs concentration, reduces school and work performance, and when associated with postnasal drip, causes chronic throat irritation and cough. Untreated, it can progress to sinusitis, otitis media with effusion, and worsening asthma.
Allergic rhinitis develops through a two-phase process: initial sensitisation (first exposure to allergen trains the immune system to produce specific IgE) and re-exposure reactions (subsequent exposures trigger immediate symptoms). Genetic atopic predisposition is the strongest risk factor. Common aeroallergens include house dust mites (Dermatophagoides species), tree, grass and weed pollens, cat and dog dander, Alternaria and Aspergillus moulds, and cockroach allergens. Urban air pollution, early antibiotic use, and reduced early-life microbial exposure (hygiene hypothesis) increase allergic sensitisation. Emotional stress affects mast cell degranulation thresholds and worsens symptom severity. Many patients find that after 6-12 months of treatment, allergens that previously always caused symptoms no longer reliably do so.
